CHARLOTTESVILLE, Va. (CBS19 NEWS) -- Sentara Martha Jefferson Hospital is announcing plans to improve community health and well-being.

Sentara wants to broaden their reach with a new Community Health and Wellness Center.

This is in an effort to address social determinants of health that impact patients.

They said the center would serve the community by improving access to care and expanding their essential services.

"Having more space is going to help us to impact more with our community. It's going to give them a place to actually come where we're having a conference room that they can come in and sit down and get education..." said Community Benefit Manager Tameka Irving.

The new center will be located at Sentara’s Outpatient Care Center on East High Street in downtown Charlottesville.

Irving said they hope to complete the center by the first of the year.
